"b irule show all" failures and abort counts

I have a simple general question that is burning me up:

 

 

In which cases do you get "fail" and "abort" counts in the output of "b irule show all" ??

 

I mean what are the conditions that creates counts in those fields ??

 

 

I searched the manuals but i couldn't find any relevant information.
